Understanding Hip Pain

The hip is a deep joint with multiple potential sources of pain, including the joint itself, the surrounding muscles and tendons, the bursa, the labrum and structures referred from the lumbar spine. Getting the diagnosis right is the most important first step.

Our treatment is tailored to the specific cause. For arthritic hips, we focus on maintaining mobility and managing load. For tendinopathies, we use a progressive loading programme. For impingement, we address the hip mechanics contributing to the problem.

Signs and Symptoms

Common Symptoms

These are the most common presentations we see. Every patient is different, and your physiotherapist will assess your individual symptoms thoroughly.

Groin or Front of Hip Pain

Deep aching or sharp pain in the front of the hip or groin, often associated with intra-articular hip problems.

Outer Hip Pain

Pain on the outside of the hip or buttock, often associated with gluteal tendinopathy or trochanteric bursitis.

Stiffness

Reduced range of motion when rotating the hip, getting in and out of a car or crossing the legs.

Pain on Walking or Standing

Discomfort that worsens with prolonged walking or standing.

Clicking or Snapping

Audible or palpable snapping in the hip with movement, sometimes painful.

Referred Knee Pain

Knee pain that actually originates from the hip, a common and frequently missed presentation.

Yes. Exercise therapy is one of the most evidence-based treatments for hip osteoarthritis. Physiotherapy can reduce pain, improve range of motion and help you stay active for longer. Many people manage hip arthritis very effectively with physiotherapy and avoid or delay surgery.

Hip osteoarthritis involves cartilage degeneration within the joint itself. Hip bursitis or greater trochanteric pain syndrome involves tendon irritation causing outer hip pain. They feel different and are treated differently, which is why an accurate assessment matters.

Post-operative physiotherapy is essential after hip replacement surgery. A structured rehabilitation programme restores strength, mobility, gait and function after the procedure.

Clicking in the hip is very common and often harmless, caused by a tendon snapping over a bony prominence. If the clicking is painful or associated with instability, it warrants assessment. Painless clicking alone is usually not a cause for concern.
